I went to the pharmacy to pick up my granddaughters meds, found out she was cancelled. Called. Found out all the annual review was mailed to an address I lived at almost 30 yrs ago. She never even lived there. Gave them correct address on 10/14/18 to send the review. Nope still no review. Called again ten days later, said he would send out a entirely new app. I told him it was for my granddaughter only, that I just needed a review so she could be reinstated. No complete application because it took six months the first time and it was their fault I never received an snnnusl review. Still no annual review. She has been sick and nope that doesn’t matter, still no annual review.
